自殺現象は複雑であり,自殺のタイプに対応した多様な自殺予防戦略が必要である。そこで本稿でわれわれは,まず自殺をめぐるジレンマに起因する自殺予防の困難性を確認する。第2に,自殺者の意志や動機に基づく定義の問題を示しつつ,行為とその痕跡に基づく定義の必要性をうったえる。第3に,我が国の自殺認定が行為論的定義に基づいて,個々の死因を分類していることを示す。第4に,行為論的定義に基づいた,デュルケームによる自殺の社会的タイプ,自己本位的自殺,集団本位的自殺,アノミー的自殺,宿命論的自殺に基づいて,自殺予防のための個人への介入の可能性を探求する。最後に,地域住民自身によるヘルスプロモーションに貢献するため,予防医学モデルに基づく精神医学による自殺予防と社会政策的な社会学的自殺予防を概観しつつ,われわれのうつ病についての啓発活動と地域社会に潜在する対自殺抵抗資源の発見を目指した地域研究に言及する。 |

Suicide is a highly complex phenomenon. We need diverse strategies to prevent any type of suicide. In this paper, first, we explain difficulty in suicide prevention in the light of the various dilemmas. Second, we question the definition of suicide based on one's will or motivation and propose a definition based on the study of one's actions and their traces. Third, we point out that in Japan each cause of death is currently classified according to the action-oriented definition (as well as the sociological one by Durkheim). Fourth, depending upon Durkheim's social types of suicide (suicide egoiste, suicide altruiste, suicide anomique, suicide fataliste), we explore chances for intervention for individuals who could commit suicide. Finally, in order to contribute to community health promotion we propose adapting the psychiatric theory based on the model of preventive medicine as well as social policy based on the sociological model to suicide prevention. In addition to our ongoing research on depression, this perspective will also be informative in identification of the salutogenic resources (factors that support human health and well-being) that are latent in the community. |
