| 内容記述 | The purpose of this study was to evaluate interobserver differences in intrapartum cardiotocograph (CTG) interpretation among 7 perinatal experts in Japan, particularly with regard to the fetal heart rate (FHR) pattern. Printed cardiotocograms (188 pages) of each 9-minute CTG chart segment of approximately the last 3 hours before delivery of 14 highrisk term pregnant women were presented to each clinician. The Japan Society of Obstetrics and Gynecology (JSOG) Standard 82 FHR pattern classifications and the 5-level risk categories were also printed on each page. Seven of the leading Japanese perinatologists were asked to interpret these cardiotocograms. The participants were requested to mark the appropriate FHR pattern type and risk category. The interobserver agreementwas analyzed using proportions of agreement and kappa scores. The average proportion of perinatologists agreeing with the majority opinion for the 5- level risk category was 74.4±4.5%. The kappa score was 0.667±0.053, which indicates a substantial agreement. The high kappa score revealed a consensus in the interpretation of the 5-level risk category by leading Japanese perinatologists. However, differences in opinion arose in the deceleration category of FHR pattern classification. |

| 内容記述 | 日本産婦人科学会(JSOG)ガイドラインを用いた分娩時胎児心拍(CTG)の理解における観察者間差異について調べた。日本人医師7名に高リスク妊婦14名の出産前3時間における9分おきのCTG(188頁)を印刷して渡した。さらに、JSOG基準の82の胎児心拍(FHR)分類および5段階リスクカテゴリーに基づき医師が適切なFHR分類とリスクカテゴリーを選択し、観察者間の一致について分析を実施した。5段階リスクカテゴリーの一致率は74.4±4.5%であった。κ係数は0.667±0.053であり、相当の合意が示された。以上より、日本の指導的な出産期医学専門産科医において、5段階のリスクカテゴリーの理解は一致していることが示された。一方で、FHR分類については差異が認められた。FHRの一致率が最も高かったのはベースラインカテゴリーにおいてであり(41.0%)、次に一致率が高かったのは変動性カテゴリーにおいてであった(35.1%)。 | |||||||||||||||||
