満田のいう非定型精神病(発作性うつ病)の同胞症例,非定型精神病(オネイロフレニア)の家族症例および感情障害の3症例と,その家族背景について報告した。3例とも何れも心因を契機にして急性に発症し,3ヶ月ぐらいで寛解または軽快するが,再発を繰り返す傾向が強い。何れも家族関係に問題があり,第1例では母親の人格障害が,第2例では家族内緊張が,いずれもいわゆる高EEを引き起こし,第3例では,"生活者としての家族機能"が障害されていた。何れの症例においても"援助者としての家族機能"と"生活者としての家族機能"が障害され,そのことが患者の再燃,再発に影響を及ぼしていることがわかった。看護者を含め精神医療に関わるものは,この様な現状を十分に把握し,患者ならびに家族の支援をして行く必要がある。 |

This paper describes 3 psychotic cases with atypical psychoses (ictal depression), atypical psychoses (oneirophrenia) and emotional disorder together with a description of their familial backgrounds. All 3 subjects had acute onsets triggered by psychological causes. In approximately 3 months, they would go into remission or relief. However, each had a strong tendency to relapse repeatedly. Family members of all 3 subjects had emotional disorders. The mother of the first subject suffered from a personality disorder. The family of the second subject was obstinate and suffered from high emotional expression (high EE). In all 3 cases, family members that provided care and support for the patients were themselves found to have emotional abnormalities, which had a negative effect on the patients and were one of the leading causes of flare-ups and relapses. Nurses and other people engaged in mental health care must acquire a better understanding of such situations and provide support for both the patients and their families. |
