ポリ塩化ビニリデン製食品包装用フィルム(サランラップ^<(R)>,以下ラップ)を用いた治療を,ステージII(いずれも創が真皮深層にまで達する),III,IV(NPUAP分類)のいわゆる「深い褥瘡」を対象に行い,軟膏とガーゼによる治療成績と比較し,ラップの創傷治癒効果について検討した。ポケットのないステージII,IIIの褥瘡では,ラップを用いた治療の方が軟膏とガーゼによる治療に比べ,よりよい創傷治癒効果を発揮した。また,両大転子に同程度の大きさ,深さおよび壊死組織量のステージIVの褥瘡が認められる患者に対し,一方にはラップを用いた治療を,他方には軟膏とガーゼによる治療を行った。すると,ラップを用いた方は,ガーゼと軟膏を施した方に比べ,著しい死腔の拡大を来す結果となった。 |

We tried to treat pressure ulcer of Stage n (all involving the deep layer of dermis), III, and IV using plastic wrap (Saranwrap^<(R)>) to examine the effect of wrapping on wound healing of pressure ulcer, compared with the effects of conventional treatment with ointment and gauze without wrapping. In pressure ulcer without pocket formation in Stage II and III, wrapping resulted in more beneficial effects on wound healing than the conventional treatment with only ointment and gauze. In a case of IV pressure ulcers on the bilateral greater trochanters, both of which the size, depth and volume of necrotic tissue were almost the same degree, we treated with wrapping on one side and without wrapping on the other side. In this case, however, the dead space within the ulcer enlarged more remarkably by wrapping compared to that of the ulcer of contralateral side. |
