当院では,2年前より入院患者の「訓練室でできる日常生活動作(以下,ADL)」と「病棟でしているADL」の差を最小限にすることを目標に作業療法士(以下,OTスタッフ)の病棟担当制を導入している。OTスタッフが「できるADL」と「しているADL」の差の原因を見極め,病棟職員(以下,病棟スタッフ)へ分かりやすく伝達し,患者の能力を把握し「できるADL」の能力を「しているADL」へ近づけることが重要であると考え,病棟業務内にスタッフとの情報交換の場を設けた。具体的には,食事と入浴時間に病棟内での訓練を設定し,病棟スタッフとの申し送り時間を設け情報の共有を実施した。OTスタッフと病棟スタッフが認識する患者のADL能力の差についてFIM (Functional Independence Measure)を用い検討した。その結果,「できるADL」と「しているADL」の差を近づけることができた。 |

At our hospital, we introduced two years ago a system in which occupational therapist (OTR) became in charge of a ward to minimize the difference between "capability activities of daily living (ADL) in the exercise room" and "performance ADL in the ward." We considered it important for OTR to ascertain the cause of the difference between "capability ADL in the exercise room" and "performance ADL in the ward," and to convey it to the ward staff in order to make the "capability ADL" come closer to "performance ADL" by correctly grasping the ability level of a patient. We provided an opportunity for the ward staff to exchange information with OTR specifically. Therefore we adopted in-ward exercises during the feeding and bathing activities and set up a regular information exchange meeting for the ward staff with OTR to share the same information. Then, we examined the difference between the patient's ADL ability recognized by the OTR staff and that by the ward staff using the Functional Independence Measure (FIM). As a result, we could make the difference of "capability ADL" and "performance ADL" smaller. |
