We experienced a case of persistent postural-perceptual dizziness (PPPD) that developed after vestibular neuritis. At the first visit, head positional nystagmus was observed to the left direction. At first, she was treated with anti-vertigo drug and vestibular rehabilitation, but they were not effective. When she was treated with selective serotonin reuptake inhibitor (SSRI), her subjective symptoms and head positional nystagmus improved. When the dose of SSRI was reduced, the subjective symptoms and head positional nystagmus got worse. Half a year later, slight head positional nystagmus remained in the left lower head position and subjective vertical visual displacement was observed, so vestibular dysfunction remained. However, her subjective symptoms almost disappeared. SSRI administration partially might improve the nystagmus findings, promoting the effect of vestibular rehabilitation, and the subjective symptoms improved faster than objective findings. We reported a case of PPPD in which SSRI treatment and vestibular rehabilitation were effective. One and a half years after the vestibular disorder, deterioration of vestibular function persisted, but subjective symptoms improved, and the general course was seemed to be favorable. The improvement of head positional nystagmus as a whole course was slightly delayed from the subjective symptoms. |
