This study aimed to clarify challenges of medical safety education by examining incidents/accidents caused by students during clinical training as part of the Basic Nursing Training II course. The number of such events over the past 5 years was 40, and their levels of influence were as follows : 0(including near-miss events and property damage) : 19 ; 1 (not resulting in actual injury, but the possibility of the event influencing patients is undeniable) : 18 ; and 3 (requiring care or treatment) : 2. The details of the Level-0 incidents were as follows : property damage : 7 ; missing records (documents) : 6 ; inappropriate reporting : 2 ; inappropriate conversations with patients : 2 ; carrying a mobile phone : 1 ; and the failure to return keys : 1. Although it is necessary to lead
students to learn basic points of medical safety and incident/accident prevention from appropriate ethical perspectives as nurses, and enhance their awareness of patient safety through education, school systems to address these issues are limited. Helping students integrate the knowledge they have acquired at school and their experiences during clinical training, and appropriately reviewing their safety and incident/accident prevention approaches in the setting of nursing practice may be challenges faculty members face. |

基礎看護実習IIにおける過去5年のインシデント/アクシデント(I/A)を分析した。過去5年のI/A件数は40件であった。影響度0(ニアミス事故および物的損害を含む)は19件、影響度1(損傷には至らなかったが、患者への影響を否定できない可能性のあるもの)は19件、影響度3(ケアまたは治療が必要)は2件であった。影響度0の19件のうち、物的損害は7件、記録紛失は6件、不適切な報告は2件、患者との不適切な会話は2件、携帯電話所持は1件、鍵の返却ミスは1件であった。3件について分析を実施した。事例Aは乳腺切除後の80歳代女性患者におけるバイタルサイン計測位置の誤りであった。職員および学生には事前に注意されていが、両者ともに失念し他の職員から指摘された。事例Bは80歳代認知症女性の入浴介助時の学生および患者の転倒であった。事例Cは80歳代認知症男性で、ベッドから車椅子への移動を学生が介助している時に負傷した。事故の要因として最も頻度が高かったのは確認不十分および不注意であった。 |
