看護技術には可視的な「行動」と不可視的な「行為(意味づけ)」という2つの側面があるが,今まで看護技術を論じる場合この点をあいまいにしていた。本論文は看護学生の看護技術に対する不可視的な「行為(意味づけ)」の構築の仕方を論じた。研究対象は近畿地方の都市近郊にある看護系短期大学1校,3回生87名。そして調査時点で臨床実習を終了した33名の「移動動作」に関する質問紙調査である。学生は臨床実習後,最も困難だと答えた項目に「患者への声かけ」を挙げていた。「患者への声かけ」行動は,看護技術の対象がモノではなく人であることの証であり,看護技術の意味づけを知るうえで重要な行動である。この理由から「患者への声かけ」に関する記述のある学生2名,記述のない学生3名の計5名を選出し看護技術の意味構築を辿っていった。その結果,その状況の定義づけ(意味づけ)が実践する個々の学生によって様々であったが,「意味づけ」の多様さは決して無秩序ではなく,看護技術の「行為論」的観点からの看護技術の「標準化」の作業は可能ではないかと考える。 |

Referring to the problem of nursing skills, one has to take notice of two aspects of actions that are necessary for performing skills:(1) visible aspect ("doing"); and (2) invisible aspects ("thinking,""feeling," or "constructing meaning"). In this paper, the authors attempted to understand how nursing students constructed the meanings out of their invisible aspects of actions and skills. The third-year students at a nursing college in a mid-western part of Japan were invited, and 87 students participated in the study. The students completed a survey concerning their experience in assisting older persons in transferring from a bed to a wheelchair. Out of the 87 students, 33 students who have completed gerontological nursing practicum reported in the survey that one of the most difficult skills was "talking to their patients." This finding provided the evidence that the students were aware of the fundamental principal of nursing that the patient is a "human being," and not a "thing." For this reason, the authors selected a group of five students who described their experience in "talking to their patients" in the survey. The authors analyzed the content of the five students' descriptions and clarified how their meaning-construction unfolded. The findings were as follows:(1) the students constructed or developed a variety of their subjective meanings or definitions out of the situations that they observed or attended as a part of the clinical practicum;(2) these meanings or definitions were not necessarily presented in a random order, but rather in a structured order to some extent. The findings indicated that standardized protocols of nursing skills could be established in the future through analysis and synthesis of nurses' and nursing students' invisible aspects of actions, or meaning-construction regarding their skills. |
