レビー小体型痴呆の患者の示す三大症状としては,緩やかに進行する痴呆,幻覚,パーキンソニズムがある。今回,私達はレビー小体型痴呆患者を受け持ったので,この様な患者の看護における要点について報告する。患者は80才男性で,幻視等の精神症状のため本院に入院となった。5年前より痴呆およびパーキンソニズムを呈していた。精神症状に対し少量のカルバマゼピンおよび塩酸チアプリドが投与された。投与数日後より他の患者との関係も改善された。痴呆とパーキンソニズムは存在したが,精神症状の改善により日常生活活動も改良された。この様な患者は時には抗精神病薬に著しく反応することがあるので,この様な点を考慮に入れて薬剤の不利な作用を厳重に観察することが重要である。 |

Patients of dementia with Lewy bodies (DLB) show three characteristic symptoms : slowly progressive dementia, hallucinations, and parkinsonism, which are the features of this disorder. We treated a patient with this disorder, and here report the key points in nursing such patients. The patient, a 80 year old man, was admitted to our hospital because of visual hallucinations and violent tendencies. He had suffered from dementia and gait disturbance for the previous 5 years. In order to control the psychiatric symptoms, small amounts of carbamazepine and tiaprid were prescribed. Hallucinations were quickly alleviated by these drugs and the relationship between this patient and other patients soon improved markedly. Although dementia and parkinsonism are still evident, the patient is able to function much better in conducting the activities of daily life since his psychiatric state ameliorated. Patients of this disorder sometimes show pronounced responses to antipsychotic drugs, and therefore it is important to take this point into account and strictly observe the adverse effects of prescribed medications. |
