同胞2名ともに満田のいう非定型精神病(発作性うつ病・発作性不安)に罹患し,頻回に再発し,入・退院を繰り返していた。同胞2名とも何れも,コタール症候群,発作性うつ病,発作性不安のほか多彩な病的体験を有し,そのため衝動的に頻回の自殺企図,自傷行為を行っていた。この様な自殺企図,自傷行為は妹の方により頻回に認められた。その際患者の表情は急に険しくなり,頻回に詰所へ症状を繰り返し訴えに来るため,看護師は,平素よりこの患者の示す発作性の症状を熟知しておく必要がある。さらに医師の指示のもと抗精神病薬の注射を行うことにより,数分後には症状が軽減し,自殺企図,自傷行為を防ぐ事が出来た。なお,母親の性格は未熟で,自己中心的,執着性,演技性の人格障害で,子供との共依存が強いため,子供が入院すると可哀相といって直ぐに退院させてしまい治療を継続することが困難であった。そのため,母親に対し子供の疾病について十分理解させ,異常行動について説明するために多大の努力を要した。 |

Two siblings had been suffering from atypical psychoses (ictal depression, ictal anxiety). The diagnosis was made according to descriptions by Mitsuda. Their symptoms often flared up, and they were repeatedly admitted and discharged. Both siblings had various pathologic experiences including Cotard's syndrome, ictal depression, and ictal anxiety, which led to frequent suicide attempts and self-mutilation. The younger sister attempted suicide more often and performed more acts of self-mutilation than the elder sister. In such situations, her face would suddenly become stern and she would repeatedly come to the nurse station to complain about her symptoms. Nurses had come to know her ictal symptoms well on a routine basis, and when her symptoms emerged, an antipsychotics was immediately administered intramuscularly according to a doctor's instruction. Her symptoms would abate in a few minutes, and we would be able to prevent her attempts at suicide and self-mutilation. Their mother had immature characteristics such as selfishness, tenacity, histrionic personality disorder. She was also strongly codependent on her children, and when one of her children was admitted to the hospital, complaining that she felt pity for her child, and would allow the child to leave the hospital. Therefore, we put considerable effort into making her understand her children's disorders and explaining her own abnormal behaviors. |
